In addition to Groopman, which is a wonderful book you should read "Checklist" by Atul Gawande, another very helpful insight into levels of care. I don't agree that community hospitals are necessarily bad, but some are worse than others, and for complex surgery, it is the volume of surgery done at that hosptial that counts, not necessarily whether it is a university or community
